Adirondack Health Institute (AHI) is a New York State-designated lead Health Home and serves all ages. A Health Home is not a place; it is a care management service that connects the member to community, social supports, and health care. Our mission is to serve as a bridge that connects the member to essential resources in the area in hopes of improving quality of life as well as member education.
AIM Services provides residential and community-based services to people with disabilities, including those with traumatic brain injuries and those who are looking for nursing home transition or diversion.
AIM is dedicated to supporting the “power of potential” in people of diverse abilities by focusing on supporting people in achieving their personal goals, while promoting self-confidence and independence.

CARES envisions a future where all people have access to safe, affordable housing and the social supports to remain housed. CARES is a non-profit organization that empowers communities to end homelessness through community planning, program data, supportive housing, and awareness-building. CARES collaborates with communities towards the creation of systems that end and prevent homelessness.
CREATE is dedicated to providing a space where the benefits of art-making impact both the individual and the community as a whole. CREATE offers low-cost art workshops and groups, community wellness activities, and open studios to promote art-making and the creative process as a vehicle for personal growth, communication, and social change.
The Legal Aid Society of Northeastern New York fights for fairness, dignity, and justice for those living in poverty and for a society which is inclusive and equitable for all. We transform lives, build community and empower people by using the law to address individual and systemic wrongs and inequities, providing free civil legal aid to low-income upstate New Yorkers across five offices in the region, serving 16 counties and the St. Regis Indian Reservation.
Mechanicville Area Community Services Center (MACSC) is committed to improve the lives of area residents through family support, empowerment, recreation and education. MACSS aims to to promote healthy relationships between families and the community at large.

The Prevention Council works to prevent alcohol, drug and other substance abuse throughout Saratoga County by providing education, information and referral services on the subjects of alcohol, tobacco, other drug and violence prevention to individuals and local communities, thereby enhancing the quality of life for all citizens by diminishing the adverse effects of substance abuse and violence.
The Salvation Army Saratoga aims to stand in the gap to provide wraparound services to children, families, and individuals from all backgrounds and stages of life, to help end intergenerational poverty through means of financial, physical, emotional, mental, and spiritual care. he Salvation Army has relied heavily on volunteers —the army behind The Army — who help fulfill our promise to do the most good.
The Saratoga County Chamber of Commerce initiates and provides programs and services to enhance and promote a healthy economy, making Saratoga County an ideal place to work, live and visit. It seeks to provide a strong, sustainable and county-wide. When tomorrow arrives, we will be ready. We’ll have flights to book, places to park and jobs to pursue.
The Saratoga Springs Planning Office reviews development projects proposed within the City, provides recommendations to City Council, and also provides a forum for public comment on issues dealing with development of our community.
VCHC has made a commitment to help homeless Veterans attain and maintain permanent housing, achieve financial stability, have access to health care and regain their dignity. The programs of Veterans & Community Housing Coalition provide a resource and ray of hope for homeless Veterans.
Wellspring provides crisis and support services for survivors of dating abuse, domestic violence, trafficking, sexual assault, and elder abuse. in Saratoga County. Wellspring also assists survivors of sexual assault in Washington County. Wellspring provides a full range of no-cost, confidential crisis and support services to any person directly or indirectly impacted by relationship or sexual abuse, as well as prevention and social change programming.
ASCEND Mental Wellness
ASCEND Mental Wellness mission is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community. ASCEND provides a range of services, including Clinical Counseling, Restorative & Supportive Residential Services, Outreach/LEAMHRS, Support Services (East Side Center Psychosocial Day Program, Dual Recovery & Wellness Garden), and Health Home Care Management.
CHS addresses the difficult issue of homelessness through several programs and services, which include; a Runaway and Homeless Youth Emergency Shelter for teens, a Street Outreach Program, a permanent supportive housing program for homeless youth and young adults ages 18 - 25, and a housing and eviction prevention programs for adults and families.
CHS strives to be a recognized leader in finding effective solutions to social issues.

Family Service Association provides help to people facing a situation that is beyond their resources by guiding them toward resolution with kindness, assistance and support. Our programs support families and individuals in Warren, Washington and Northern Saratoga counties. Our Special Needs Program provides financial assistance, referrals and resources to those experiencing homelessness or in danger of becoming homeless. We provide essential personal care items and have an emergency food pantry. We have been providing programs and support to this region for 115 years.
LifeWorks Community Action is a community of neighbors helping neighbors build bright and stable futures in Saratoga County and the Northern Capital Region. We take a whole family approach to service coordination and offer a variety of programs to help address challenges like hunger, childcare, utility costs and language barriers. Over the past 60 years, these programs have helped thousands of families and beyond meet their immediate needs and work toward economic security.

Saratoga County Department of Mental Health & Addiction Services mission is to provide outpatient mental health and alcohol/drug treatment services that are accessible and responsive to the needs of persons suffering from mental illness and/or alcohol/drug problems, their families, and the community.

SOS provides shelter, housing and food stability services for adults experiencing homelessness in the Saratoga region. Programs include Code Blue, street outreach, year-round emergency shelter, care management, and supportive housing.
Southern Adirondack Independent Living (SAIL) is dedicated to empowering individuals to live more independently within their communities. Our services include independent living support, health insurance navigation, transition and waiver assistance, an equipment loan program, and more.
Soldier On’s mission is to help veterans reclaim their place in the community while returning meaning, dignity, and hope to their lives. Soldier On offers a continuum of ongoing care to veterans and their families that includes long-term housing with an array of support services delivered where they live. The goal is to provide homeless or at-risk homeless veterans with permanent, supportive, sustainable housing with services.
WAIT House empowers homeless and at-risk youth and young families across the North Country to find safety, stability, and hope. Based in Glens Falls, we provide emergency shelter, transitional living, street outreach, and Family Opportunity Center services that prevent homelessness and strengthen families. Through compassionate care, advocacy, and community collaboration, WAIT House helps youth and families rebuild their lives and create brighter, more independent futures.
